PLEASANT GROVE, Utah — October 1, 2026 — New Department of Defense clinical guidance is putting testosterone health into sharper focus for U.S. service members. Under the updated policy, male service members age 30 and older will be screened for testosterone deficiency as part of periodic health assessments, while younger male service members may request screening or be evaluated when clinical indicators are present.
The Defense Health Agency says screening begins with a structured review of symptoms and risk factors. When testing is indicated, testosterone levels are measured on two separate mornings under standardized conditions. If a deficiency is confirmed, service members and their military medical providers can discuss treatment options, which may include testosterone replacement therapy or other approaches based on individual circumstances. Treatment is not automatic, and the Department has emphasized that testosterone therapy is intended to address documented deficiency rather than serve as a performance enhancer.
Adam Russell, PA-C, lead practitioner at Hormone Balance Centers, said the new policy may encourage more men to pay attention to symptoms they have previously dismissed.
“Testosterone levels are only one part of the picture, but having a structured screening process can help start an important conversation,” Russell said. “The right next step is a careful clinical evaluation that considers symptoms, repeat lab testing, overall health, fertility goals, and the risks and benefits of treatment before any therapy is started.”
Hormone Balance Centers has published a new resource explaining the military screening program, what service members should expect from testing, and questions to discuss with a qualified medical provider. Learn more about military testosterone testing and TRT.
The military policy also reflects a broader shift toward identifying hormone-related health concerns earlier while using standardized clinical criteria. Hormone Balance Centers encourages service members to use their military health system for required screening and to seek individualized medical guidance for any treatment decision.
